Job Title

In collaboration with The Institute for Urban Care Program Coordinator, responsible for the provision of intensive case management services provided by a violence prevention/intervention grant program that provides bedside visits and intensive home-based follow-up services to a target population who have been hospitalized for violent injuries at Capital Health. Conducts psychosocial assessments and provides psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds. Provides counseling and/or psychotherapy services to individuals, groups, and families. Provides consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Evaluates own practice, and direct reports practice, through participation in a professional peer review case conferences or other organized means. Implements treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical and social problems to achieve treatment goals. Demonstrates a working knowledge of professional standards and ethics regarding the delivery of clinical social work services. Note: This is a grant-funded position, and employment is subject to the continuation of financial support from the grant program.

Minimum Requirements

Education: Master's degree in social work. Licensed Clinical Social Worker (LCSW) required

Experience: One year of clinical experience in social work.

Other Credentials: Social Worker - Licensed

AHA BLS. Non-Licensed must obtain before end of orientation period

Knowledge and Skills: Possesses excellent organizational, interpersonal, conflict resolution, and verbal and written communication skills. Ability to manage multiple patients/families simultaneously and to respond quickly in a fast-paced environment. Possesses a willingness to work with the programs' unique patient population. The expectation of this position requires full-time on-site work

Special Training: Familiar with Microsoft Excel, PowerPoint, and Outlook.

Mental, Behavioral and Emotional Abilities: Must be comfortable working with diverse age ranges.

Essential Functions

  • Provides intensive case management, mentoring, and advocacy to target populations involved in violence (hospitalized for violent injuries). Makes the connection with current trauma victims and their families enrolled in the violence prevention program to link them with supportive services.
  • In collaboration with the Program Coordinator, provides client referrals to community service providers based on established guidelines.
  • Conducts psychosocial assessments and provides psychosocial treatment to a wide variety of individuals from various socioeconomic, cultural, ethnic, educational, and other diverse backgrounds.
  • Assesses the psychosocial functioning and needs of patients and their family members and formulates and implements appropriate treatment plans. Conducts mental status examinations and formulates DSM-5 related diagnoses.
  • Identifies patients' problems, strengths, weaknesses, coping skills, and assistance needed, in collaboration with the patient, family, and interdisciplinary treatment team.
  • Uses medical and mental health diagnoses, disabilities, and treatment procedures, to include acute, chronic, and traumatic illnesses/injuries, common medications, and their effects/side effects.
  • Implements treatment modalities in working with individuals, families, and groups experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals.
  • Provides counseling and/or psychotherapy services to individuals, groups, and families. Must practice within the bounds of the license or certification.
  • Represents the patients' needs and rights to providers and the community in a variety of settings.
  • Provides crisis intervention and case management as needed.
  • Maintains appropriate charting; documents progress notes and treatment plans as required by the clinic protocol and other mandated regulatory agencies; prepares correspondence regarding social evaluation of patient and patient follow-up.
  • Actively participates in clinical team meetings
  • Performs other duties as required.

Continuous physical demands include: Talk or Hear. Lifting Floor to Waist 15 lbs. Lifting Waist Level and Above 10 lbs. Sensory Requirements include: Accurate Near Vision, Accurate Far Vision, Color Discrimination, Minimal Depth Perception, Accurate Hearing.

Anticipated Occupational Exposure Risks Include the following: Bloodborne Pathogens, Airborne Communicable Disease.
