Home Health Case Manager - Registered Nurse
REQUIRED \t - 1 year of RN experience required \t
- Current State issued Registered Nurses License\t
- Require personal transportation, current state issued driver's license, good driving record and auto insurance as required by law.\t
- 1+ year working in home health or geriatric patient setting
PREFERRED \t - Bachelor's degree in nursing\t
- 3 years of health care experience or experience working with geriatric patients \t
- Bi-lingual

The Aurora PACE center opened in 2003. Aurora has a successful rehab walking program. Historic Aurora artwork is displayed in the hallways and the participant lounge, which also features computers and comfortable reclining chairs. The center features a winding hallway that participants use to walk loops (17 times around equals one mile).
Participants enjoy weekly activities including bingo and Red Hat Society outings. Travel with Marjorie, a monthly program, focuses on learning about destinations near and far such as the Arctic, Ireland and the Colorado National Monument.
The Aurora PACE center is one of six InnovAge PACE Centers in Colorado. Other centers are located in the communities of Denver, Lakewood, Northern Colorado, Pueblo, and Thornton.
